Please expect the assessment process to take 45 minutes, up to two hours, once you are seated with a clinician. We suggest filling out paperwork ahead of time to help expedite processes.
What should I bring with me?

Insurance card: Medicare, Medicaid and private insurance accepted. Self-pay based on a sliding scale is accepted as a form of payment for services. All are served regardless of ability to pay.

Proof of income: COMCARE provides a sliding fee scale based on household income. Please bring verification of your household income which may include pay stubs or benefit statements.

Identification: Bring a copy of your identification as well as identifying information for youth coming into services. Examples include Driver's License, Social Security Card, School ID, Military ID, or other photo ID.

Proof of Guardianship: If you are bringing a child to an assessment and you are not the biological parent, we must have proof of guardianship. If you do not have one, please use the accepted Power of Attorney Document. The document MUST be notarized.

Mental Health Records: If you have records available, please bring them to the intake. We also recommend talking to your other providers to release health records and sign a release of information. This information will help us coordinate your healthcare.
Same Day Assessment Considerations: Intakes will be offered on a first come, first served basis. Open Access Intakes are not guaranteed. If we have reached our scheduling capacity, options to complete an intake will be provided.
Intake Documents
Please complete the documents listed under the Same Day Assessment Program, you intend to utilize. After the documents are submitted, you will receive a call providing additional information to guide you through this process. If you are unable to submit documents online, you may go to any COMCARE location and documents will be provided to you.

Adult Mental Health Treatment Services
To initiate services, please complete the Adult Intake Packet and return electronically. If you are unable to complete electronically, an intake packet may also be picked up on-site, Monday-Friday, 8 a.m. - 5 p.m.

Adult Substance Abuse Treatment Services
To initiate services, please complete the Addiction Treatment Intake packet and return electronically. If you are unable to complete electronically, an intake packet may also be picked up on-site anytime, Monday through Friday, 8 a.m-5 p.m.
Once the completed forms are received, you will be contacted by a staff member to schedule an intake appointment.
