COVID-19 Vaccination Reporting

Sedgwick County is asking vaccine providers for their assistance to help track the number of residents who have been vaccinated against COVID-19 and the number of doses received. This is a voluntary system. Data will be reported in aggregate on the Sedgwick County COVID-19 Vaccine StoryMap on Wednesday each week.

All agencies and organizations providing COVID-19 vaccinations in Sedgwick County are encouraged to report. Providers that choose to report should, at a minimum, report vaccination doses by noon on Tuesday for the previous whole week (Sunday to Saturday) to ensure timely comparison. Numbers submitted after that deadline are still accepted and will be reflected in the next weekly update, even if from previous weeks.

Instructions to complete form.

  1. Enter the Vaccine Dose Reporting Information for the previous whole week (Sunday to Saturday) (preferred). Providers can report previous data, as well.
  2. If the provider has multiple locations, the data can be added across all locations and reported in one submission or can be reported by location.
  3. For vaccination monitoring timeliness, report by noon on Tuesdays. Numbers submitted after that deadline are still accepted and will be reflected in the next weekly update.
